product gift - implementation?

I need to sell a gift product as a reward. And I see in the tables Cart and Order field is_gift. But I do not see their functionality. I thought these fields should be in the proper itemsCart and itemsOrder right?
At the moment, the gift id is saved to the session and I somehow need to save it in the order with 0 price. I would be glad to help. Thank you.
